Ian Callaghan? He was in the World Cup winning squad but only appeared once in a qualifying game so did'nt think you got a medal for that?

His main successes were with Liverpool obviously and got promotion with them from div.2.

He won successive promotions with Swansea and I presume his 4th div promotion was with Crewe?

He also won the European Super Cup and the Cup Winners Cup but I guess that would have given it away

Edit: he wouldnt need Crewe cos he got 2 with Swansea and one with Liverpool
