Bad news indeed.

What happens in situations like this? We've got him on loan and i assume paying a big percentage if not all of his wages. Does an insurance policy covering both clubs kick in to cover the wages element? Who pays for the initial operation?

Probably defined in the terms of his contract. I'd imagine the club have some kind of medical insurance.
